Woem health is responsible for processing personal data as reflected in this privacy statement. Processing of personal data is done carefully in accordance with the requirements of the General Data Protection Regulation (AVG) from 25 May 2018.

For proper treatment, it is necessary for me as your treating therapist to create a file. This is also a legal duty imposed by the Medical Treatment Agreement Act (WGBO/Wkkgz). Your file contains notes on your state of health and data on the examinations and treatments carried out.

The file also contains data that are necessary for your treatment and that I have requested from another healthcare provider, such as your GP, after obtaining your explicit consent.

I do my best to ensure your privacy. This means, among other things, that I:

• Carefully handle your personal and medical data,
• Ensure that unauthorised persons do not have access to your data.

As your treating therapist, I am the only one who has access to the data in your file.

I have a legal duty of confidentiality (medical confidentiality).

The data in your file may also be used for the following purposes:

• To inform other healthcare providers, for example when the therapy is completed or in case of a referral to another practitioner.  This is done only with your explicit (written) consent.
• For the use for observation, during my absence.
• For anonymised use during peer review.
• A small part of the data from your file is used for financial administration, so that I, or my administrator can prepare an invoice.

If I want to use your data for any other reason, I will inform you first and ask for your explicit consent.

You have the right to inspect your client file at any time.
You have the right to have your data deleted at any time.
As required by law (WGBO), the data in your client file will be kept for 20 years.

PRIVACY ON THE HEALTHCARE INVOICE

The care bill you receive contains the details requested by the health insurer so that you can claim it from your health insurer.

• Your name, address and place of residence.
• Your client number and/or date of birth.
• Insurance and/or policy number.
• The date of the treatment.
• A brief description of the treatment.
• Performance code.
• The cost of the consultation.

COOPERATION WITH OTHER ORGANISATIONS

From the practice, I cooperate with the following organisations in relation to processing your data and have entered into a processing agreement with them:

• MyDiad, digital patient/client administration.
• E-bookkeeping, financial administration.
• Ramaje studio, web administrator.

COOKIE POLICY

What are cookies?
The Woem health website uses cookies. A cookie is a simple small file sent with pages from this website and stored by your browser on your computer's hard drive. The information stored therein can be sent back to our servers during your next visit.

Use of permanent cookies 

A permanent cookie allows you to be recognised when you visit the website again. As a result, the website can be set specifically to your preferences. Even if you have given permission for cookies to be placed, we can remember this by means of a cookie. This saves you having to keep repeating your preferences, thus saving you time and making our website more enjoyable to use. Permanent cookies can be removed via your browser settings.

Google Analytics cookies 

Through this website, a cookie is placed by the US company Google, as part of the "Analytics" service. I use this service to track and get reports on how visitors use the website. Google may provide this information to third parties if Google is legally required to do so, or insofar as third parties process the information on Google's behalf. I have no influence on this. I have not allowed Google to use the obtained analytics information for other Google services. The information Google collects is anonymised as much as possible. Your IP address is expressly not provided. The information is transferred to and stored by Google on servers in the United States. Google states that it adheres to the Safe Harbor principles and is affiliated with the Safe Harbor programme of the US Department of Commerce. This implies an adequate level of protection for the processing of any personal data.

Cookies Social Media 

This website includes buttons to promote ("like") or share web pages on social networks such as Facebook and Instagram. These buttons work by means of pieces of code originating from Facebook and Instagram themselves. Cookies are set by means of this code. I have no influence on these. Read the privacy statements of Facebook, Instagram and Google (which may change regularly) to read what they do with your (personal) data that they process via these cookies. The information they collect is anonymised as much as possible. The information is transferred to and stored by Facebook, Instagram and Google on servers in the United States.
Facebook, Instagram and Google claim to adhere to Safe Harbor principles and are members of the US Department of Commerce's Safe Harbor programme. This means there is an adequate level of protection for the processing of any personal data.

I also work with Facebook pixels through which we match the information and content I share and ads I use to your interests based on your website visit.

Woem health may amend this privacy statement. New versions are always published on the

website. We therefore advise you to consult this statement regularly so that you are kept informed of any changes.

Privacy verklaring Woem (NL)

Woem health is verantwoordelijk voor de verwerking van persoonsgegevens zoals weergegeven in deze privacyverklaring. Verwerking van de persoonsgegevens gebeurt op zorgvuldige wijze in overeenstemming met de eisen die de Algemene Verordening Gegevensbescherming (AVG) vanaf 25 mei 2018 stelt.

Voor een goede behandeling is het noodzakelijk dat ik als uw behandelende therapeut een dossier aanleg. Dit is ook een wettelijke plicht opgelegd door de Wet op de geneeskundige behandelingsovereenkomst (WGBO/Wkkgz). Uw dossier bevat aantekeningen over uw gezondheidstoestand en gegevens over de uitgevoerde onderzoeken en behandelingen.

Ook worden in het dossier gegevens opgenomen die voor uw behandeling noodzakelijk zijn en die ik, na uw expliciete toestemming, heb opgevraagd bij een andere zorgverlener bijvoorbeeld bij de huisarts.

Ik doe mijn best om uw privacy te waarborgen. Dit betekent onder meer dat ik:

• Zorgvuldig omga met uw persoonlijke en medische gegevens,
• Ervoor zorg dat onbevoegden geen toegang hebben tot uw gegevens.

Als uw behandelende therapeut heb ik als enige toegang tot de gegevens in uw dossier.

Ik heb een wettelijke geheimhoudingsplicht (medisch beroepsgeheim).

De gegevens uit uw dossier kunnen ook nog voor de volgende doelen gebruikt worden:

• Om andere zorgverleners te informeren, bijvoorbeeld als de therapie is afgerond of bij een verwijzing naar een andere behandelaar.  Dit gebeurt alleen met uw expliciete (schriftelijke) toestemming.
• Voor het gebruik voor waarneming, tijdens mijn afwezigheid.
• Voor het geanonimiseerde gebruik tijdens intercollegiale toetsing.
• Een klein deel van de gegevens uit uw dossier wordt gebruikt voor de financiële administratie, zodat ik, of mijn administrateur een factuur kan opstellen.

Als ik vanwege een andere reden gebruik wil maken van uw gegevens, dan zal ik u eerst informeren en expliciet uw toestemming vragen.

U heeft te allen tijde recht tot inzage in uw cliëntendossier.
U heeft te allen tijde het recht uw gegevens te laten verwijderen.
De gegevens in uw cliëntendossier blijven, zoals wettelijk vereist (WGBO), 20 jaar bewaard.

PRIVACY OP DE ZORGNOTA

Op de zorgnota die u ontvangt staan de gegevens die door de zorgverzekeraar gevraagd worden, zodat u deze nota kan declareren bij uw zorgverzekeraar.

• Uw naam, adres en woonplaats.
• Uw klantnummer en/of uw geboortedatum.
• Verzekering en/of polisnummer.
• De datum van de behandeling.
• Een korte omschrijving van de behandeling.
• Prestatiecode.
• De kosten van het consult.

SAMENWERKING MET ANDERE ORGANISATIES

Vanuit de praktijk werk ik samen met de volgende organisaties met betrekking tot het verwerken van uw gegevens en ben ik hiermee een verwerkingsovereenkomst aangegaan:

• MijnDiad, digitale patiënten/cliënten administratie.
• E-boekhouden, financiële administratie.
• Ramaje studio, Webbeheerder.

De website van Woem health maakt gebruik van cookies. Op deze website zijn buttons opgenomen om webpagina’s te kunnen promoten (“liken”) of delen op sociale netwerken als Facebook en Instagram. Deze buttons werken door middel van stukjes code die van Facebook en Instagram zelf afkomstig zijn. Door middel van deze code worden cookies geplaatst. Ik heb daar geen invloed op. Lees de privacyverklaring van Facebook, Instagram en Google (welke regelmatig kunnen wijzigen) om te lezen wat zij met jouw (persoons)gegevens doen die zij via deze cookies verwerken.
